By filling out this form, I (as parent or guardian) agree to allow my child to volunteer in a Good News Club® and/or a 5-Day Club®, ministries of Child Evangelism Fellowship® Gwinnett Chapter from August 1, 2023, to July 31, 2024. I understand that I am responsible for my child's transportation to and from activities.

Sometimes we use photos taken during Good News Club and 5-Day Club's activities on our website and for other publicity purposes.  Your "yes" below gives us permission to use your child's picture.

I hereby assign and grant to Child Evangelism Fellowship Inc., its subsidiaries, successors, and church partners, and assign the unqualified right to the ownership, use, and proceeds of all photographs of my child, without reservation or limitation, including use of photographs of my child for advertising and promotional purposes and for the use of our church partners.
