By filling out this form, I (as parent or guardian) agree to allow my child to attend Good News Club after school at Beaver Ridge Elementary School for the school year 2025-2026. I understand it is my responsibility to pick up my child immediately after club ends and failure to do so will jeopardize my child's continued participation. 

Due to an update to the Telephone Consumer Protection Act, we must receive a written confirmation in order to send text messages. Your typed name below is your written agreement.
