By filling out this form, I (as parent or guardian) agree to allow my child to attend Good News Club after school at Silver City Elementary School for the school year 2025-2026. I understand it is my responsibility to pick up my child immediately after the club ends, and failure to do so will jeopardize my child's continued participation. 

Due to an update to the Telephone Consumer Protection Act, we must receive a written confirmation to send text messages. These are only used for your club updates and information.  By signing this form, you are permitting us to text your phone number. You may opt out at any time, but realize you will not receive your club updates.
