By filling out this form, I (as parent or guardian) agree to allow my child to attend Good News Club after school at Puckett's Mill Elementary School for the school year 2025-2026. I understand it is my responsibility to pick up my child immediately after club ends and failure to do so will jeopardize my child's continued participation.
